Important Information!

Completing this form does not guarantee an actual consultation appointment. You will be contacted by the library with a confirmation of your consultation appointment time.

Provide as much information as possible regarding your research or information need.
Information provided by the requester is used only to help the librarian efficiently prepare for consultation appointments.
Consultations must be scheduled 48 hours in advance of appointment to allow the librarian time to prepare.
Consultations provided by way of phone or chat are primarily recommended for Distance Learners.
Appointments are limited to an 1-hour slot & may be shorter during weekends, summers, break times, etc...
Arriving more than 10 minutes late may cause you to lose your appointment.
If you are unable to make it to an appointment on time, or need to cancel, please contact the Library at (859) 572-5457 or via email as soon as possible.
